Three FSA members have been nominated for these positions. Please use the form below the candidates’ Statements of Interest to vote for TWO (2) of the following three candidates. You may only vote once, and please use the email that is associated with your FSA membership to ensure that your vote is valid.

Jonathan Hadaway, MD
It is an honor to be considered for the position of Central District Director of the Florida Society of Anesthesiologists. I was involved with the FSA during my residency training at the University of Florida, having attended and presented at the annual meeting. I have also been involved in advocacy at the national level through the American Society of Anesthesiologists. I began in medical school, where I chaired the anesthesia interest group, was seated at the Medical Student Component House of Delegates and worked to increase awareness of the specialty of anesthesiology.
I am currently an associate anesthesiologist with JLR/USAP in Orlando. For 2016-17 I will be representing the FSA as an alternate delegate to the ASA.
If chosen for the position of District Director, I will work hard to represent the physician anesthesiologists of Central Florida. I believe my background and close ties to my residency program, as well as my current employment with a large, active group make me well-suited for this position. Thank you.
Jodi Bosak, MD
I am interested in being an FSA Central District Director. I believe I can contribute by offering enthusiasm and energy. I am involved in multiple leadership roles within my group and institution, and I am looking forward to more diverse roles.
My undergraduate degrees are from Penn State University. I have a Masters of Biological Science from Drexel University. My medical degree is from Penn State College of Medicine. I completed my residency at Penn State Hershey Medical Center, as well as a Pediatric Fellowship. I am Board-Certified in Anesthesiology and Pediatric Anesthesiology. I came to Florida five years ago to join the University of Florida as an Assistant Professor of Anesthesiology where I did mostly Pediatric Anesthesia. After a year, I moved to the Orlando area to join Anesthesiologists of Greater Orlando, a Sheridan group. Currently, I spend most of my time at Winnie Palmer Hospital for Women and Babies, doing about 75% Obstetric Anesthesiology.
I have been involved with the ASA, SPA, and SOAP, so I am looking forward to being more active in the FSA. This year, I was elected to become an Alternate Delegate to the ASA and to the FMA. I am participating in Sheridan’s Emerging Leaders program. I have also participated in Orlando Health’s Physician Leadership Academy. Presently, I am also honored to be the only female in my group’s partnership track. I am intimately involved with the daily scheduling of nearly 20 physicians and approximately 40 CRNAs/AAs at 8 different sites.
Moeed Azam, MD
Thank you for the opportunity for service in the Central District, especially with Legislative affairs. It has been rewarding to have participated with the ASA and FSA over the past few years with Congressional lobbying trips in DC, involved with fundraising efforts at the state and federal levels, and to have actively engaged our legislators on the VA Care issue. Building upon important relationships with our Central Florida Congressmen, including Mica and Webster, and it has been rewarding to advance our issues and gain their signatures of support. I look forward to continuing to work locally with the all the members of the FSA leadership, as we have done together on the federal level.
Practice and Background: Dr. Azam joined JLR Medical Group after completing his anesthesiology residency training at The Johns Hopkins Hospital and a medical internship at University of Miami’s Jackson Memorial Hospital. Dr. Azam graduated from Rutgers University – New Jersey Medical School. He has served as the Director of Liver Transplant Anesthesiology since the program’s inception at Florida Hospital over a decade ago. He serves on the Clinical Quality Committee, and IT committee, the Clinical Governance Board of JLR Medical Group, and has been involved in several initiatives including: Blood Management, Regional Anesthesia programs, development of the Cerner EMR, and PSH.
