Exhibitor Appointed Contractor Form– Due May 16, 2025
This form is required if a third-party company will be installing and dismantling your booth. A certificate of insurance must be provided as well.
Exhibitor Profile Form – Due May 16, 2025
Company information provided in the profile will be published in the meeting program.
Giveaway Notification Form– Due May 16, 2025
Please complete and submit this form if you plan to distribute promotional materials to attendees from your booth.
Pre-Registration Mailing List Order Form ($100) – Due May 16, 2025
Lists will be emailed approximately one month prior to the meeting and will include pre-registrants names, institutions and mailing addresses. Email addresses are not released. Data will only be provided for doctors who opt-in to share their information.
Badge Registration Form – Due May 16, 2025
Booth fees include full registration and all food functions for four representatives. There will be a charge of $200 for each additional representative.
EXHIBIT HALL LOCATION AND PACKAGE
The exhibits will be located in the Ponce de Leon Ballroom 4-6, adjacent to the general session room. The exhibit fee includes:
» One 8′ x 10′ pipe & drape booth
» One 6 ft. draped table
» Two chairs
» One wastebasket
» One identification sign
» Four representative name badges

The service kit contains shipping instructions, important deadlines, material handling fees, rental options for custom booth furniture & fixtures and more.
SHIPPING DIRECTLY TO THE BREAKERS – NOT ADVISED
Exhibitors are strongly encouraged to ship their booth freight through PME Expo, NOT directly to The Breakers. Your freight will be delivered right to your booth, helping to avoid delays or misdirected freight. Material handling charges will be applied regardless of whether you ship through PME Expo or directly to The Breakers.
If you choose to ship or deliver freight directly to The Breakers, you will need to use the link below to submit a form, providing details about boxes/crates, freight deliveries, and booth set-up. After completing the form, you will be directed to The Breakers Payment Vault to enter your credit card details. You will need to include #1500 as the required number when entering the payment information into the the Breakers Payment Vault.
